I think that you are missing a crucial step on Medium - reporting abusers. When a commenter steps over the line on one of my articles, I always report them to Medium, then block them.

It is my understanding that Medium will delete user accounts that consistently post abusive comments.

My guess is that commenters who abuse you also abuse other authors. They will not stop until Medium steps in and deletes their accounts.
